One Hazardous Duty Award Medal from Public Health Service Commissioned Corps. The planchet is bronze and circular in shape. At the center of the obverse is a caduceus crossed with a horizontal foul anchor. Around the edge of the face are the embossed words "U.S. Public Health Service." The reverse is stamped "Hazardous Duty Award." A small suspension ring attaches the medal to a tapered ribbon. The ribbon is alternating stripes of white, maroon, white, blue.

The Public Health Service Hazardous Duty Service Award is presented to a service member of the PHS for service in a position that frequently places an officer's safety at risk. This service must be for a prolonged period of time of 200 exposure hours over a 180 consecutive day period.
