Obverse: Five pointed bronze star tipped with trefoils containing a crown of laurel and oak. In the center is Minerva, the Roman goddess of wisdom and warfare, standing with left hand resting on fasces and right hand holding a shield blazoned with the escutcheon of the United States. She repulses discord, represented by a male figure holding snakes. It is made of solid red brass, oxidized and buffed.

Reverse: Engraved "For Heroism / in / Rescuing Shipmates / Overcome / In Double Bottoms / By Fumes of Turpentine / Robert Klein / Chief Carpenter's Mate U.S.N / USS Raleigh / January / 25 / 1904"

Medal is attached to ribbon bar with a gold anchor.

Citation for Award: For extraordinary heroism in the line of his profession while serving on board the U.S.S. Raleigh at Olongapo, Philippine Islands, for heroism in rescuing shipmates overcome in double bottoms by fumes of turpentine, 25 January 1904.
